Backflow Prevention Device Test & Maintenance Report The Backflow Prevention Device Test & Maintenance Report available here is to be used when reporting device test results to Denver Water. This form is available in two formats to enable the addition of a company logo, name, address, etc. These files contain images of the test report form and can be used to create printed forms. They are not data entry forms; to create a data entry form, you need your own form tool and the image can then be used as an overlay. The body of the form should not be changed.
